ABSTRACT

OBJECTIVE: To compare the effect of remote fetal monitoring and outpatient monitoring of high-risk pregnancy.METHODS: A total of 222 pregnant women delivered in Shengjing Hospital of China Medical University from December 2017 to December 2018(44 cases of hypertensive disorder complicating pregnancy,40 cases of gestational diabetes mellitus,22 cases of placental abnormalities,20 cases of preventive cervical cerclage,16 cases of scar uterus pregnancy,12 cases of umbilical cord around neck for 3 weeks or more,6 cases of multiple pregnancy,10 cases of twin pregnancy with one fetus dead in uterus,52 cases of patients without complications)were divided into two groups.In the observation group the fetus was monitored remotely at home,while the control group went to the hospital for fetal heart monitoring.The monitoring effect and neonatal status of the two groups were compared.RESULTS: The incidence of neonatal mild asphyxia in observation group and control group was 1.80% vs.4.50%,and there was no severe neonatal asphyxia.There was no significant difference in the incidence of premature delivery,neonatal weight or vaginal delivery rate(P>0.05).CONCLUSION: Compared with monitoring in outpatient department,remote fetal monitoring can detect abnormalities in time and intervention can be performed,so it is equally effective for high-risk pregnancy.

ABSTRACT

This paper discussed the use of glucocorticoids in amniotic fluid embolism treatment by analyzing the pathophysiological basis of amniotic fluid embolism and the pharmacological effects of glucocorticoids,and expounded the viewpoints and controversial issues supporting the application of glucocorticoids in amniotic fluid embolism.The conclusion is that there is no evidence to support the conventional use of glucocorticoids in the treatment of amniotic fluid embolism,although there is theoretical basis.It is controversial to use or not use large doses of glucocorticoids.

ABSTRACT

OBJECTIVE: To investigate the feasibility of the application of autologous umbilical cord blood transfusion in intrapartum neonatal surgery.METHODS: From August 2008 to December 2018,15 cases of birth defects diagnosed in Shengjing Hospital affiliated to China Medical University who had received prenatal or neonatal surgery and had cord blood retained for autologous transfusion were selected.Routine biochemical tests were performed on the retained cord blood and the peripheral blood of the children before transfusion,follow-up was conducted on the postoperative infants with autologous blood transfusion,and blood routine tests and other relevant postoperative indicators,such as length of hospital stay and duration of intravenous nutrition support,were tested.RESULTS: Comparing the routine blood test of umbilical cord blood of the fetuses with that of the peripheral blood of neonates,there was no obvious statistical difference in the number of red cells[(4.15 ± 0.35)× 1012/L,(4.39 ± 0.31)× 1012/L,P=0.069],erythrocyte deposited[0.4749±0.047,0.5072±0.0367,P=0.052],thenumberofhemoglobin[(156.67±13.28)g/L,(166.47±13.73)g/L,1012/L,P=0.391].No adverse reactions were observed after umbilical cord blood transfusion.After transfusion,hemoglobin reached the predetermined indexes,and no second transfusion was performed.CONCLUSION: The index of autologous umbilical cord blood is basically the same as that of neonatal peripheral blood,and it is simple,easy and cheap,which avoids the adverse reaction of allogeneic adult blood transfusion;there was no adverse prognosis.Therefore,it can be used for neonatal blood transfusion preparation.

ABSTRACT

[Objective]We aimed to explore the diagnostic value of DNA fragmentation index(DFI)and acrosin activi-ty in male infertility.[Methods]Nine hundred and two semen samples were collected from patients and assessed for the DNA fragmentation index by sperm chromatin dispersion test,acrosin activity,as well as standard sperm parameters according to the WHO criteria. Statistical analysis was performed with SPSS.[Results]Statistically significant differences were observed in age,sperm concentration,total motility,progressive rate and acrosin activity among different group of sperm DNA dam-age(≤10%,11~20%,21~30%,≥31%).Sperm acrosin activity also showed difference in sperm concentration,total num-ber,total motility,progressive rate,normal morphology rate,teratozoospermia index(TZI)and sperm DFI.The DNA frag-mentation rate and sperm concentration,sperm motility,forward motility rate,total sperm count and acrosin activity was neg-atively correlated,while it is negative correlatd with TZI. Acrosin activity and sperm concentration,sperm motility,forward motility rate,sperm count and normal morphology rate was positively correlated,while it is negative correlated with the ab-normal rate of head,sperm deformity index,DNA fragmentation rate.[Conclusion]Sperm DNA damage and acrosin activity could partly reflect the quality of sperm. Moreover,sperm DFI maypredict the sperm motility part while the acrosin activity more likely related to sperm morphology.

ABSTRACT

AIM: To investigate the efficacy and safety of low dose mitomycin C (MMC) to prevent haze in trans photorefractive keratectomy (TransPRK) with moderate and high myopia, and to observe the changes of corneal density.METHODS: Sixty-one patients underwent TransPRK with moderate and high myopia.Eyes were divided into research group (0.1g/L MMC for 40s) and control group(0.2g/L MMC for 40s) randomly.There were 21 patients in research group and 40 patients in control group.Cornea epithelial healing time, pain score, visual acuity, manifest refraction, haze and cornea density were analyzed.RESULTS: The epithelial healing time (0.1g/L group: 3.86±1.11d, 0.2g/L group: 4.23±1.27d) and pain score (0.1g/L group: 2.01±0.58, 0.2g/L group: 1.79±0.7) were no significant difference between two groups(P=0.667, P=0.582).It was similar in spherical equivalent at 1mo and 3mo post-operation(0.1g/L group: 0.28±0.25, 0.05±0.23D;0.2g/L group:-0.13±0.17, 0.07±0.22D;P=0.178, P=0.490).The BCVA of control group decreased at 1mo and improved to the same level as pre-operation at 3mo(F=15.847, P<0.001);0.1g/L group showed the same trend, but the changes were no significant difference(F=3.038, P=0.093).There were also no significant difference in Haze between two groups post-operation(z=-0.709, P=0.479;z=-0.478, P=0.633).The change of cornea density was matched with the BCVA (0.1g/L group F=27.399, P=0.001;0.2g/L group F=8.313, P=0.001)and it was similar between two groups.CONCLUSION: The using of low dose MMC to prevent haze in TransPRK with moderate and high myopia is safe and effective.It is therapeutic equivalence to regular dose (0.2g/L).Besides the slit lamp, we can use the corneal density to measure the corneal transparency.

ABSTRACT

This study is to observe the protection effect of amentoflavone (AMT) in Selaginella tamariscina against TNF-alpha-induced vascular inflammation injury of endothelial cells. On the basis of TNF-alpha induced human umbilical vein endothelial cell, observe the influence of AMT on endothelial active factor, the contents of SOD and MDA, the protein expression of vascular endothelial adhesion molecules and inflammatory factor; study the effect of its common related signal pathways such as NF-kappaB; research the effect of AMT against TNF-a induced human umbilical vein endothelial cell injury by means of MTT, ELISA, Western blotting and the cell immunofluorescence. The results showed that AMT could increase the content of NO and decrease the levels of VCAM-1, E-selectin, IL-6, IL-8 and ET-1; enhance the activity of SOD, reduce the content of MDA; downregulate the protein expressions of VCAM-1, E-selectin, NF-kappaBp65 and up-regulate IkappaBalpha, attenuate the NF-kappaBp65 transfer to cell nucleus. AMT has the effect of protect vascular endothelial and maybe via the signal pathway of NF-kappaB to down-regulate the inflammation factor and oxidative damage factor of downstream.

ABSTRACT

<p><b>BACKGROUND</b>Vascular rings are uncommon anomalies in which preferred strategies for diagnosis and management may vary among institutions. In this study, we reported our approach and a review of our 5-year experience.</p><p><b>METHODS</b>From May 2006 to April 2011, 45 children (31 boys) with vascular rings underwent surgical repair at Beijing Children's Hospital. Nineteen patients (26%) had associated heart anomalies.</p><p><b>RESULTS</b>There were two hospital deaths. At follow-up, 11 patients still had intermittent respiratory symptoms, but these symptoms had no effect on growth or physical activities. No patients required reoperation.</p><p><b>CONCLUSIONS</b>The rates of misdiagnosis and missed diagnosis of vascular rings are higher than those of other congenital heart diseases. A high index of clinical suspicion coupled with the use of computed tomography enables early diagnosis. Surgical repair can be performed successfully, although a number of patients will have persistent symptoms.</p>

ABSTRACT

<p><b>OBJECTIVES</b>To investigate total drinking water intake of adults in the four cities of China in summer.</p><p><b>METHODS</b>A total of 1483 adults aged 18 - 60 yrs from Beijing, Shanghai, Chengdu and Guangzhou were selected using multiple-stage random sampling method. The information of amounts and types of daily drinking water was recorded by subjects for seven consecutive days using a quantitative measurement. The amounts and types of daily drinking water among different cities and between men and women or urban and rural was analyzed.</p><p><b>RESULTS</b>The median of daily total drinking water of subjects was 1488 ml, with significant difference among the four cities (1579, 1793, 1150, 1467 ml in Beijing, Shanghai, Chengdu and Guangzhou city, respectively, χ(2) = 154.31, P = 0.000). The median of daily drinking water was significantly higher in men (1679 ml) than women (1370 ml) (Z = 8.34, P = 0.000), but no significant difference was found between urban (1514 ml) and rural (1466 ml) daily drinking water median (Z = -0.81, P = 0.420). The median of daily consumption of plain water, tea and beverages were 786, 109, 186 ml, respectively. Among four cities, the highest consumption of plain water was in subjects of Guangzhou (917 ml), while the highest tea consumption in Shanghai (257 ml) and the highest beverages consumption in Shanghai (323 ml) and Beijing (264 ml). Consumption of tea in men (229 ml) was significantly higher than that in women (57 ml) (Z = 7.52, P = 0.000). Subjects in urban (693 ml) had lower consumption of plain water than those in rural (914 ml). The proportion was 32.4% (480/1483) for subjects with water drinking less than 1200 ml per day.</p><p><b>CONCLUSION</b>The daily consumption of total drinking water, including plain water, tea and beverages is different in adults among different cities and is different in gender and regions. It is nearly 1/3 of subjects with daily total drinking water less than current Chinese recommended water intake (1200 ml).</p>

ABSTRACT

<p><b>BACKGROUND</b>The basic helix-loop-helix transcription factor HAND2 plays an essential role in cardiac morphogenesis. However, the prevalence of HAND2 mutations in congenial heart disease (CHD) and the correlation between the HAND2 genotype and CHD phenotype have not been studied extensively.</p><p><b>METHODS</b>We amplified the exons and the flanking intron sequences of the HAND2 gene in 131 patients diagnosed with congenital defects of the right ventricle, outflow tract, aortic artery or cardiac cushion and confirmed the mutations by sequencing.</p><p><b>RESULTS</b>Seven mutations including three missense mutations (P11R, S36N and V83L), one isonymous mutation (H14H) and three mutations in untranslated region (241A > G, 604C > T and 3237T > A) were identified in 12 out of the 131 patients. Both nonisonymous mutations are located in the transcriptional activation domain on the N-terminus. Only one mutation (S36N) was identified in 250 normal healthy controls. The distribution of 3637T > A is the unique one which was different between the 2 groups.</p><p><b>CONCLUSIONS</b>HAND2 may be a potential candidate gene of stenosis of the right ventricle, outflow tract. Further study of those with a family history of HAND2 mutations will help convincingly relate their genotype to the pathogenesis of CHD.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the potential of siRNAs targeting sphingomyelin phosphodiesterase 1 (SMPD1) in protecting the oocytes from apoptosis, and explore new approaches to female fertility preservation.</p><p><b>METHODS</b>Chemically synthesized siRNA targeting SMPD1 were introduced into mouse oocytes retrieved by hyperstimulation, and the cell apoptosis was analyzed by comic assay 48 and 72 h later.</p><p><b>RESULTS</b>In the oocytes without any siRNA injection, oocyte DNA damage occurred after 24 h, and large amount of DNA fragments migrated from the cells 48 h later. In oocytes injected with siRNA003, DNA migration decreased significantly as compared with the control and the other two groups injected with siRNA001 and siRNA002 (P<0.01).</p><p><b>CONCLUSION</b>siRNA targeting SMPD1 may protect the oocytes from apoptosis, and has the potential for use in future female fertility preservation.</p>

ABSTRACT

<p><b>BACKGROUND</b>Congenital heart disease is a diverse group of diseases determined by genetic and environmental factors. Considerable research has been done on genes associated with development of the heart. A recent focus is the role of transcription factor TBX5 in the development of atria, left ventricle and conduction system. As part of a larger study, high density, single nucleotide polymorphism (SNP) scanning was used to explore the relationship between TBX5 gene polymorphism and susceptibility to ventricular septal defect not associated with forelimb malformation in the Chinese Han population.</p><p><b>METHODS</b>One hundred and ninety two paediatric patients with congenital ventricular septal defect and 192 matched healthy control subjects were studied. The haplotype reconstructions were calculated by PHASE2.0 software. Haploview software was used to perform linkage disequilibrium assessment and defining of haplotype blocks. The algorithm used for defining of blocks was the confidence interval method.</p><p><b>RESULTS</b>The TBX5 gene region can be divided into 3 haplotype blocks of 27, 15 and 2 SNPs. Strong linkage disequilibrium exists within each block. SNP rs11067075 within the TBX5 gene had significant correlation with ventricular septal defect (P = 0.0037) by single marker association analysis. In addition, a 20 kb haplotype composed of 27 SNPs correlated with ventricular septal defect (P = 0.05, multiple loci regression analyses based on reconstructed haplotype blocks).</p><p><b>CONCLUSIONS</b>TBX5 is associated with the occurrence of ventricular septal defect and may be a predisposing gene to congenital heart disease in Han Chinese. This finding has set a direction for further genetic and functional studies.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the mechanisms of intestine absorption of astragaloside IV in rat.</p><p><b>METHOD</b>The K(a) and P(app) of astragaloside IV was investigated using single-pass intestinal perfusion technique in rats. HPLC was used to determine the concentration of astragaloside IV. The effect of absorption site, drug concentration and the inhibitors of P-glycoproteon on the absorption had been studied.</p><p><b>RESULT</b>By the testing of the statistics, the K(a) and the P(app) values of the duodenum, jejunum, ileum, colonic had significant differences (P < 0.05). The concentration from 20-80 mg x L(-1) had no distinctive effect on the K(a) and P(app) of small intestine. The inhibitors of P-glycoproteon had no distinctive effect on the absorption of small intestine.</p><p><b>CONCLUSION</b>Astragaloside IV is absorbed by typical passive diffusion mechanism.</p>

ABSTRACT

A rapid LAMP detection method with primers designed on genus-specific region identified in the gyrA gene was established in this assay. All four Campylobacter jejuni from different sources were detected positive and fourteen non Campylobacter bacteria were negative, which shows excellent specificity of the primers. Compared with plate count and PCR method, the LAMP method and the PCR method had equal sensitivity, which were three orders of magnitude higher than plate count. In this assay, we also found out that the treatment of DNase could reduce the dead bacteria DNA effectively. The LAMP detection on chicken indicated relatively good result on detection of Campylobacter jejuni combining with treatment of DNase.
